That's funny I usually back hand my husband in the shoulder. Other people have even asked me to back hand him.
Like the time when the check out lady asked if we wanted the milk in a bag and he said no leave it in the carton!.....She asked me to smack him, and as we were leaving she wanted me to smack him again.
She knew him in high school and apparently he hadn't changed!!
Unfortunately either of these responses will indicate 'Yes', unless you answer all questions with 'NOYFB'. Since it IS none of their business, and has no bearing on your medical care (unless you have lead poisoning...), is it wrong to just put 'No'? I don't think so.
