My father-in-law was petrified of shots and received his immunizations the same way! He could type when he entered the Air Force so one of his duties was office work where the personnel records were also kept. When it came time for overseas deployment and the necessary shots, he "checked" his files to be sure his shot record was up to date. He was also a commercial artist so he made sure his record had the proper signature by the doc.
