With only one incident, it's difficult to diagnose. I always make sure the rounds are loaded tight to the back of the magazine. I wonder if it's possible the first round drug the second round forward. They always do but perhaps in this case it was too much? Maybe a rough case, hard to say.
I've never replaced a magazine spring in my life and never had an issue. I'm not to dedicated to replacing recoil springs for that matter. I'd shoot more and see if it happens again.
