    Default P45 Off-Center Hit

I just bought a Kahr P45. Quickly found that it failed to fire at least two out of 6 rounds. The Primer hit is way Off Center and seems to hit light. Is this common for a Kahr?

    Not common at all.

    New or used?

If used I would disassemble the striker assembly to see if the striker is broken or the channel is full of crud.

If new I would call Kahr.

I'd want to know a bit more, you might be experiencing the smear as I call it. The strike isn't a simple hit but a hit and then a drag, this is very common and no concern.
I would still go with the above and clean the striker channel and look things over.
On the rounds that didn't fire, what did the primers look like?

    Cue sound of Head slap.

Being off centered is also a signal that it might have been out of battery a small amount. Do I remember this correctly?

Yes, you do and it could also explain the light hits.

What brand of ammo? IIRC, Kahr's 45's have a relatively tight chamber and may cause issues with some ammo. I know mine don't like Corbon ammo.
